You’ve spent so much time caring (read: obsessing) about what they need that you’ve lost sight of what you need.
Are you asking too much?
Constantly walking on eggshells?
You find yourself saying "yes" when you want to say "no," yet the thought of setting boundaries (or even knowing what “boundaries” are) feels overwhelming.
You dream of speaking up with clarity and confidence, but self-doubt, guilt, and fear hold you back.
The echoes of past experiences, if you’re an adult child of an alcoholic or “ACoA”, had a dysfunctional family, or past addicted relationships, repeat in your mind. You are having trouble trusting your own instincts.
Sound familiar?
Hi, I’m Ayla Fleming
I’m here to help you stop the codependency cycle of pleasing and over-functioning in your relationships, regain clarity, & strengthen your voice. Maybe, you’re gaining these skills for the first time.
If you want to finally live a peaceful life, whether you want to stay with your partner or not, disconnect from your family, stay in relationship with your parents, or keep that job- it is key that you learn the emotional skills to feel justified in your boundaries and let go of the anxiety to control other adults’ responses, actions, and behaviors.
Therapy with me is a collaborative journey where we build a strong foundation of trust and mutual respect, creating a safe and accepting space for your transformation. I get it, because I have been there too.

Why choose
Ayla Fleming Psychotherapy?
I help women understand how factors like over-functioning/ under-functioning dynamics in relationships, over-using substances, sibling order, attachment issues (anxious or avoidant, vs. secure), a history of child abuse or neglect in the family of origin, “compulsion to repeat”, other traumas, enabling, gossip, and triangulation, can sometimes set the stage for a toxic cycle that keeps individuals and families “stuck”
It can be deeply difficult to stop participating in old dynamics. It may even feel like you’re abandoning your family (called “Survivor’s Guilt”). However, it only takes one person in the system to change to have an impact.
Therapy can help you let go of guilt and shame. It can also help you learn to side-step previously automatic patterns of responding to others’ immature behavior.
I cultivate a warm therapeutic relationship to help you achieve meaningful, lasting change within both yourself and your relationships- regardless of whether others decide to change with you.
As an intentionally micro-boutique practice, I offer personalized attention and care far beyond what larger clinics can provide.
I develop a tailored plan with each individual, couple, or family and we modify it ongoing as needed. Treatment completion means you feel fully confident to implement the skills independently.

Frequently asked questions

I work with highly successful women, couples, and families affected by their own—or a loved one’s—substance use or mental health challenges. Many of my clients have achieved great success but struggle with unhealthy patterns in relationships and want to create a healthier legacy than the one they grew up with.
I help clients:
Repair strained communication
Reduce over-functioning that creates unbalanced relationships
Address avoidant tendencies like estrangement
Manage guilt around setting boundaries or distancing from family
We focus on learning to set and maintain healthy boundaries without guilt, which allows for safer, more vulnerable communication. Boundaries also help practice unconditional love—even with a partner struggling with addiction or mental illness—and create the space to reduce contact with loved ones who disrespect them.
Our work may include taking temporary “healthy distance,” understanding difficult feelings tied to ending relationships, and preventing the patterns of your family of origin from repeating in your life. For women who experienced childhood abuse or neglect, setting boundaries can be especially challenging, as compliance may have been necessary to survive.
Many of my clients are high-achieving entrepreneurs or professionals. For those in the public eye, privacy is a priority, and I take it very seriously.

Sessions are via video through a secure, HIPAA-compliant platform. Remote counseling provides convenience and privacy. If you prefer in-person, please note that I expect to have an office in Center City, Philadelphia within the next year.

I do not accept insurance. However, I do not want finances to be a barrier to you getting help. I can educate you about “out-of-network” benefits if you have them. Or, I am sometimes able to offer a reduced rate.
Some of my clients use their out-of-network insurance benefits to get reimbursed for part of the session. If that’s important to you, it’s a good idea to check with your insurance company before scheduling, because reimbursement rates can vary a lot.
Most of my clients choose to pay cash to avoid dealing with insurance companies, which can be tricky—and sometimes even require sharing medical records.
One of the benefits of working with an out-of-network provider is more privacy and discretion. I take confidentiality seriously and make it a priority to protect your information.
If you’d still like to try submitting for reimbursement, I can provide a receipt for you to submit to your insurance company. I just don’t communicate directly with them.
Highly specialized providers often go out-of-network because of their training and their commitment to keeping clients’ information private.
